Please complete the survey you’ll find when you click the button below. Follow the instructions at the top of the survey and make your own copy.

When you’re all done with the survey, please send an email to [email protected] to let us know and we’ll get started setting up your policies. If you have questions while filling out the survey, feel free to email that same address.

My New Policies

Depending on your website, your policies will include the Dynamic Privacy Policy and one or more of the following additional policies:

• Terms of Service to protect YOU from your customers by stating the rules for using your website.
• Disclaimer to limit your liability if you sell or provide legal, health or other advice or sell third-party services or products on your site.
• Cookie Policy and Cookie Consent so users can choose their consent settings (required by GDPR and a number of US state privacy laws).
• EULA (end user license agreement) to limit liability for businesses that sell software.

Consent checkboxes will also be added to website forms and linked to your new privacy policy.

Don’t forget: send an email to [email protected] when you’re done with your survey, along with your Word doc or link to your survey on Google Docs, and we’ll get started setting up your policies.
